Patient Centered Medical Home

Plymouth Pediatric Associates is proud to serve as a medical home for our patients.  A medical home is a model of primary care that is patient-centered, comprehensive, team-based, coordinated, accessible, and focused on quality and safety. Patient Centered Medical Home has become a widely accepted model in primary care and is a philosophy of health care delivery that enables our patients and families to have stronger relationships with their clinicians and care teams.

In addition to our clinicians and care teams, the medical home program at Plymouth Pediatric Associates includes registered nurses who serve as Medical Home Coordinators. They function as advocates for patients and families by removing barriers to care, ensuring smooth transitions of care and decreasing care fragmentation. This program ensures that care is received in the right place, at the right time, and in the manner that best suits a patient’s needs.
